Warren Barr Oak Lawn is seeking a Restorative Nurse (LPN or RN) to join our skilled nursing facility in Oak Lawn, IL. You will oversee the restorative nursing program, evaluate resident progress, and contribute to care plans to ensure residents receive high-quality, compassionate care.

Qualified applicants must hold an Illinois RN or LPN license, CPR certification, and have at least two years of RN/LPN experience in a Skilled Nursing Facility, including one year in restorative nursing.

Qualifications

• Minimum 2 years RN/LPN experience in a Skilled Nursing Facility.
• Minimum 1 year Restorative Nurse experience.

Responsibilities

• Oversee and supervise the restorative nursing department.

• Perform resident assessments and contribute to care plans.
• Assist in admission and discharge planning and documentation.
• Complete portions of MDS and other required records.

As a Restorative Nurse, you are the backbone of the Clinical Department. Your work will ensure our residents receive the high standard of care they have grown to expect by overseeing and supervising all aspects of the restorative nursing department, evaluating residents restorative nursing programs, performing resident assessments, participating in the resident admission and discharge planning, contributing to resident care plans and completing portions of MDS and other documentation. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

• As a minimum, an unencumbered State of Illinois RN or LPN license;
• C.P.R certification.
• At least two (2) years of RN or LPN experience in a Skilled Nursing Facility setting;
• One (1) year prior experience as a Restorative Nurse is required.
