Job summary

Elan Skilled Nursing in Scranton, PA is seeking a Restorative LPN to join our dedicated team. The role focuses on coordinating restorative nursing programs to promote residents' independence and well-being, in collaboration with the interdisciplinary team.

Responsibilities include developing care plans, monitoring progress, and ensuring documentation and compliance with regulations. Licensed Practical Nurse credentials and CPR certification are required, with prior nursing experience preferred.

Qualifications

• Graduate of an accredited School of Practical Nursing.
• Current, unrestricted LPN license for the state of employment.
• Current CPR certification required.
• Minimum one (1) year of nursing experience preferred.

Responsibilities

• Coordinate, implement, and monitor restorative nursing programs.
• Develop individualized restorative care plans with the interdisciplinary team.
• Collaborate with Physical, Occupational, and Speech Therapy to promote continuity of care.
• Educate residents, families, and staff regarding restorative goals and interventions.
