Job summary

Ellen Memorial Rehabilitation and Healthcare Center LLC is seeking an experienced LPN Licensed Practical Nurse to join our team. In this role you will provide compassionate patient care under RN supervision in a warm, home-like environment.

Responsibilities include administering medications, monitoring vital signs, assisting with mobility, and collaborating with an interdisciplinary team to develop individualized care plans. Pennsylvania LPN licensure is required.

Qualifications

• Current LPN license in the state of Pennsylvania.
• Ability to lift, transfer, and reposition patients; walk and stand for long periods.
• Strong communication and interpersonal skills; ability to work effectively in a fast-paced environment.

Responsibilities

• Participate in the delivery of quality patient care, under the supervision of the Registered Nurse, to a diverse population of patients
• Administer medications, treatments, and therapies as ordered by the healthcare provider
• Conduct vital signs, perform basic wound care, and assist with mobility and ambulation
• Develop and implement individualized care plans, in collaboration with the interdisciplinary team
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
• Assist with daily living activities, such as bathing, dressing, and grooming, as needed
• Participate in quality improvement initiatives to ensure optimal patient outcomes
