Job summary

Apploi seeks a Restorative Aide in the Chicago area to coordinate, implement, and provide restorative nursing care for patients under Nursing and Rehabilitation guidance. The role focuses on maximizing physical functioning and emotional wellness through restorative programs.

The incumbent will oversee restorative initiatives, communicate with Nursing and Rehabilitation staff, assist in transitions to restorative care, and maintain comprehensive documentation and patient progress.

Qualifications

• Registered CNA by the State.

Responsibilities

• Oversees, directs and participates in restorative programs for individual patients.
• Communicates with the Director of Nursing and Rehabilitation Nurse regarding goals and approaches for patient needs.
• Supports transition of patients from skilled rehab to restorative programs.
• Attends rehab rounds and updates staff on patient rehab/restorative status.
• Maintains restorative nursing progress and documentation for all participants.
• Visits patients daily to ensure program relevance and productivity.
• Trains caregivers in restorative techniques and assists in care delivery.
• Monitors ROM and T&P orders for compliance.
