Job summary

The Registered Nurse (Restorative Coordinator)/Community Living Center (CLC) position at the VA focuses on coordinating care around patient education, self-management, and satisfaction across the continuum of care. It includes administering medications per policy and supervising nursing staff to maintain high-quality restorative care.

The role emphasizes collaboration with OT/PT, development of functional goals for Veterans, and ongoing progress monitoring with monthly adjustments to therapy

Qualifications

Experience in restorative nursing and care coordination.
Ability to supervise licensed nursing staff and coordinate care teams.
Proven ability to educate patients and families about care plans.

Responsibilities

Coordinate care with focus on education, self-management and satisfaction.
Administer medications and procedures per policies.
Supervise restorative personnel as appropriate.
Lead nursing education, competencies and quality improvement initiatives.
Develop measurable functional goals for Veterans.
Collaborate with OT/PT for admissions and goal setting.
Monitor progress and adjust therapy/goals monthly.
Perform other duties as listed in the functional statement.
