Job summary

The U.S. Department of Veterans Affairs seeks an experienced Registered Nurse Patient Care Coordinator to oversee and coordinate patient care, acting as a clinical resource, liaison, and advocate for patients and families.

You will facilitate care planning, monitor progress, and ensure continuity across services and settings. Responsibilities include collaborating with the interdisciplinary team, supporting safe care delivery, and guiding transitions.

Qualifications

• English language proficiency as required by VA standards.
• Graduate of a school of professional nursing approved by ACEN/CCNE or equivalent; or bridge program pathway with licensure.
• Current, full, active RN license in a U.S. state, territory, or D.C.; GNT exception may apply during licensure pursuit.

Responsibilities

• Oversee and coordinate patient care activities to ensure safe, effective delivery.
• Serve as clinical resource, liaison, and patient advocate among patients, families, and interdisciplinary teams.
• Support care planning, monitor patient progress, and ensure continuity of care across services.
• Facilitate communication among healthcare providers and other stakeholders.
• Assist with care transitions and ensure adherence to standards of quality and safety.
