Job summary

RYZE at the RIDGE is seeking a Restorative Aide to coordinate and provide restorative nursing care under the Nursing department. The role emphasizes collaborating with Rehabilitation and Nursing staff to maximize patients' physical functioning and emotional well-being.

The candidate must hold an active CNA license and have at least 1 year of experience. This in-person position requires daily patient contact and participation in rehab rounds and care planning discussions.

Qualifications

• Must be a registered CNA by the State.

Responsibilities

• Oversees, directs and participates in restorative programs for individual patients.
• Communicates with the Director of Nursing and Rehabilitation Nurse regarding goals and approaches.
• Facilitates transition of patients from skilled rehab to restorative programs.
• Maintains restorative progress and documentation for all patients.
• Trains primary caregivers in restorative techniques and proper device usage.
• Ensures compliance with ROM and T&P orders.
• Assists in patient/family communication about progress and care planning.
