Job summary

Pivot Rehabilitation is seeking a Restorative Aide to support restorative and rehabilitative care for ProCare I-SNP members. Location opportunities include Park Valley & Falcon Ridge or Bel-Air & San Gabriel facilities.

The role involves assisting with personal care, restorative tasks, and promoting patient independence. Travel to additional SNF locations within the geographic area is required.

Qualifications

• State certification as a Nursing Assistant is mandatory.
• Ability to read, write, and speak the English language.
• Must be available to work six days per week
• Ability to travel to additional SNF locations within geographical area

Responsibilities

• Provide personal care to patients in a manner conducive to their safety and comfort.
• Assist in the provision of restorative or rehabilitative care.
• Performs restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, etc., within training scope.
• Promotes and supports the greatest possible degree of independence for patients.
• Advises Director of Rehab (DOR) of Patient's status daily.
• Contributes knowledge of Patient's conditions/needs to patient care plans.
• Assists in the implementation of care plans as directed within policy and procedure.
• Observes and reports on Patients' conditions and documents observations as required.
