Job summary

Pawnee Mental Health Services is seeking dedicated staff to support individuals with mental health challenges across home, school, work, and community settings. You will participate in crisis response, provide one-to-one support, and help clients achieve person-centered goals while ensuring safety and confidentiality.

Responsibilities include documenting progress, cooperating with treatment teams, and assisting with daily activities and community outings.

Responsibilities

• Monitor and document client progress in the chart, providing direct services in locations such as home, school, work, crisis/respite house or other community settings.
• Provide one-to-one support to clients in crisis to help maintain stability and avoid more restrictive placements.
• Communicate client strengths, needs, progress to family and treatment team to reinforce strategies.
• Implement scheduled day activities including exercise, socialization, goal setting, illness management and community outings as appropriate.
• Review client incidents with supervisor and submit reports per policy and procedure.
• Advocate for client rights and community integration, maintaining dignity and confidentiality.
