Qualifications

• Bilingual preferred for effective client communication.
• Experience coordinating care and supporting clients toward independence.

Responsibilities

• Maintains a caseload as determined by CMHA and funder.
• Provides direct care including education, skills training, recreation, and safety monitoring.
• Teaches life skills to live independently, supports finances and daily living activities.
• Educates clients to utilize emergency and community resources.
• Develops and reviews individualized treatment plans with clients.
• Ensures timely documentation per CMHA policies (notes, plans, authorizations).
• Supervises self-administration of medication when applicable and documents it.
• Collaborates with CMHA teams to ensure integrated services.
• Communicates with external providers; may provide transportation.
• Performs other related duties as assigned.
