Job description

Position Purpose:

Provides resource support to members and connects members to local resources for various services (e.g., employment, housing, independent living, justice, foster care) based on care/service assessment and plans. Works with leadership to identify the best, most appropriate local resources for members' needs based on industry best practices.

Location:

Remote - NC Region 5 counties: New Hanover, Brunswick, Columbus, Pender, Bladen, Robeson, Scotland, Sampson, Montgomery, Moore, Lee, Hoke, Cumberland, Harnett, Richmond.

Key Responsibilities:
  1. Assist in assessing member needs and contribute to the development of care plans/service plans aligned with members' goals and preferences, including referral coordination for services such as employment, housing, independent living, justice, foster care, and community transitions.
  2. Connect members to local resources, assist with documentation, and support application processes as needed.
  3. Act as a resource for members seeking support, determining eligibility and care needs based on assessments and plans.
  4. Monitor care/service delivery, perform follow-ups to ensure timely access and transitions.
  5. Collaborate with providers, community organizations, stakeholders, and government entities to facilitate access to essential services.
  6. Develop education and training programs for staff and providers to enhance resource identification.
  7. Assist in drafting materials and developing policies, procedures, and programs related to local resource services, including advocacy and emergency housing.
  8. Ensure all resource information is documented and current.
  9. Conduct outreach to hard-to-reach individuals and families in the community.
  10. Perform other duties as assigned to meet member and business needs.
  11. Comply with all policies and standards.
Qualifications:

Bachelor's degree required with 2-4 years of relevant experience, or equivalent experience demonstrating applicable knowledge, duties, scope, and skill level.

Compensation and Benefits:

Pay Range: $22.50 - $38.02 per hour

Centene offers a comprehensive benefits package including competitive pay, health insurance, 401K and stock purchase plans, tuition reimbursement, paid time off and holidays, and flexible work arrangements (remote, hybrid, field, or office).

Actual pay will be based on skills, experience, education, and other factors. Total compensation may include additional incentives.
