Job summary

Brighton/Suncrest Hospice is seeking a Registered Nurse to provide compassionate, patient-centered hospice care to a group of patients visiting private homes, SNFs, and assisted living facilities.

As a valued member of the interdisciplinary team, you will contribute psych-social assessments, coordinate care, and travel throughout the service area, including Walnut Creek, Oakland, Alameda, and surrounding counties. A current RN license and reliable transportation are required.

Qualifications

• Current Registered Nurse (RN) license in good standing in the state of practice.
• A passion for providing exceptional, patient-centered hospice care.
• Strong collaboration and communication with interdisciplinary care teams.
• Ability to work independently while effectively managing your daily schedule.
• Excellent time management and organizational skills to balance patient visits, documentation, meetings, and urgent needs.
• Reliable transportation, a valid driver's license to travel throughout the service area.

Responsibilities

• Provide care for a group of patients visiting private homes, SNFs, and assisted living facilities.
• Contribute psych-social assessments and interventions as part of the interdisciplinary team.
• Cover territories including Walnut Creek, Oakland, Alameda, Solano County, and surrounding areas.
• Utilize electronic medical record (EMR) systems for documentation and care coordination.

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Resource role.

Candidate must be willing to cover all terrorities including Walnut Creek, Pinole, Oakland, Alameda, Solano County, Tri-Valley, Napa, Moraga, Martinez, Brentwood, etc.
