Job summary

Suncrest Hospice seeks a Registered Nurse (RN) to provide patient care across home and facility settings, collaborating within an interdisciplinary team. The role may operate in Case Management, Admissions, or Resource capacities, delivering psychosocial assessments and community resource knowledge.

Candidate must hold active RN licensure, be able to travel, and adeptly manage visits and documentation using an EMR system. Strong teamwork and communication skills are essential.

Qualifications

• RN licensure in good standing in state of practice
• Team-based collaboration with nurses, social workers, chaplains, volunteers, and CNA leadership
• Ability to manage visits and documentation with strong time-management skills
• Experience with or ability to learn an electronic medical records system
• Willingness to travel across the service area with reliable transportation and valid driver's license

Responsibilities

• Provide care and services for a group of patients visiting their residence or care facilities
• Contribute to the interdisciplinary team with psych-social assessments and interventions
• Work in Case Management, Admissions, or Resource roles as needed
• Coordinate with patient care team and external providers to ensure quality hospice care
• Maintain accurate documentation and timely communication via phone, email, and meetings

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Case Management, Admissions, or Resource role.

Responsibilities & Qualifications

• Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
• Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Work closely with patients on a 1-1 level but get strong support from a patient care team.
• Can work independently and on your own from a day-to-day basis with activities which include phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
• Time management skills to manage visits, documentation, meetings (i.e. to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
• Ability to learn an electronic medical records system.
• Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area
