Job summary

Brighton/Suncrest Hospice is seeking an experienced Registered Nurse to provide compassionate hospice care in patients' homes, facilities, and communities. You will collaborate with social workers, chaplains, CNAs, and volunteers to build personalized care plans that address physical, emotional, and spiritual needs.

Qualifications

• Current Registered Nurse (RN) license in good standing in the state of practice.
• Ability to collaborate with interdisciplinary care teams to develop and implement care plans.
• Strong time management and organizational skills to balance patient visits, documentation, meetings, and urgent needs.

Responsibilities

• Provide compassionate, individualized hospice care to patients wherever they call home.
• Collaborate with an interdisciplinary team to develop and implement patient care plans.
• Maintain EMR documentation and ensure timely, accurate patient records.
• Manage patient visits, travel within service area, and prioritize patient needs.
• Support quality of care with low clinician caseloads and high staff-to-patient ratios.

As a Registered Nurse (RN), you'll provide compassionate, individualized hospice care wherever patients call home—including private residences, skilled nursing facilities, and assisted living communities.

You will collaborate with an interdisciplinary team of nurses, Social Workers, Chaplains, CNAs, and volunteers to develop care plans that support the physical, emotional, and spiritual needs of patients and their families. We prioritize quality over quantity by maintaining lower clinician caseloads and higher staff-to-patient ratios. This allows our nurses to spend more meaningful time with patients and families, build lasting relationships, and provide the compassionate, personalized care every patient deserves. Depending on your strengths and interests, we have various nursing opportunities available.
