Qualifications

Responsibilities

• Performs holistic assessment of patient needs across physical, psychosocial, spiritual, and education domains.
• Develops and communicates plan of care using SBAR and collaborates with the healthcare team.
• Delivers evidence-based nursing actions, delegates appropriately, and ensures safety and infection prevention.
• Evaluates patient outcomes and updates care plans accordingly.
• Documents nursing process accurately in the electronic medical record and maintains competency.
