Job summary

Utah's Promise is seeking a Service Navigator to assist Utahns in navigating the social care system. The role involves handling calls, emails, and in-person inquiries, collaborating with cross-functional teams, and supporting clients to access essential resources.

Responsibilities include building partnerships with service providers, maintaining accurate databases, and attending outreach events. The position is primarily in-office with a hybrid Friday remote shift during regular business hours.

Qualifications

• Graduation from high school or equivalent.
• Minimum two years of experience serving clients in social services or related field OR two years post-secondary education in social work, human services, or related field.
• Proficiency with word processing, spreadsheets, and electronic calendaring programs.

Responsibilities

• Through telephony, text, chat, and other formats, help clients navigate social services and assist clients in accessing essential resources with a “whatever it takes” approach.
• Apply creative problem solving to overcome barriers when clients face crises or standard referrals are not enough.
• Ensure resolution by consistently following up with clients to confirm services were accessed and needs were met.
• Support high-risk and crisis situations using crisis intervention and de-escalation skills.
• Advocate for access and equity by championing clients facing systemic barriers to promote fair access to services.
• Maintain data accuracy, quality, and confidentiality in documentation.
