Job summary

Qualifications

• Bachelor’s degree in Human Services or related field.
• 3 years’ experience in case management or discharge planning with older adults.
• Proficient with Microsoft Office (Word, Excel, Access, PowerPoint).
• Strong customer service and communication skills.

Responsibilities

• Assess seniors and families to identify needs and connect to resources.
• Coordinate referrals, center activities and education programs.
• Maintain resource materials and data related to referrals.
• Build relationships with hospitals, healthcare providers and community partners.
• Support curriculum development, policies and strategic planning for the Center.
