Job summary

Hartford HealthCare in Newington, CT, is seeking a Healthy Aging Navigator to oversee and manage care transition operations for older adults within an assigned region.

You will serve as a lead representative of the Hartford HealthCare Community Network, ensuring timely access to services, reducing readmissions, and promoting consistency across settings. This role supports planning and program expansion within the Healthy Aging Center.

Qualifications

• Bachelor's Degree in Health & Human Service Field (Social work, Gerontology) required or strongly preferred
• Five years working in Healthcare and Care Coordination
• Exceptional Communication and Organizational skills
• Five years working with older adults
• Able to travel to locations throughout the region on a regular basis
• Strong analytical skills, able to work with and interpret large amounts of data

Responsibilities

• Oversee and manage care transition operations for older adults within an assigned region of the Hartford HealthCare system.
• Serve as a lead representative of the Hartford HealthCare Community Network within the region and ensure timely access to appropriate services.
• Reduce avoidable readmissions and improve consistency in transitions of care across clinical and community-based settings.
• Support operational planning, performance monitoring, and program expansion in alignment with organizational priorities.
