Job summary

The Village for Families and Children in Hartford, CT seeks a compassionate Care Coordinator / Community Resource Specialist to support clients through the treatment journey and link them with needed services. You will coordinate care, assist with benefits enrollment, and advocate for diverse populations.

Responsibilities include resource navigation, documentation, and collaboration with healthcare providers and community partners to improve outcomes and access to care.

Qualifications

  • Bachelor's degree in Social Work, Human Services, or a related field preferred.
  • Candidates with an Associate's degree and relevant human services experience will be considered.

Responsibilities

  • Engage clients from referral through treatment and coordinate care and services.
  • Assist clients with benefits eligibility determination and enrollment in public and private insurance programs.
  • Conduct social needs screenings and identify barriers to care.
  • Provide resource referrals and service navigation assistance.
  • Support clients and families in accessing clinical, behavioral health, and community-based services.
  • Develop and maintain strong relationships with community agencies, referral sources, and service providers.
  • Facilitate community resource linkage and coordinate referrals.
  • Maintain accurate client records, service data, and required reports.
  • Ensure HIPAA compliance and documentation.
  • Participate in interdisciplinary team meetings and care planning.
  • Contribute to quality improvement initiatives and program development.
  • Provide ongoing support, education, and advocacy for clients and families.
