Job summary

Adirondack Health is seeking a Resource Navigator to connect patients with health care coverage, primary and preventive services, and social resources. You will become an expert on programs in your service area, addressing the unique needs of patients, families, and the surrounding community.

Responsibilities include guiding clients through enrollment processes, coordinating with care teams, and ensuring access to community services and patient education to support overall well-being.

Qualifications

• Associate degree preferred.
• Administrative assistant experience preferred, strong computer skills required (Word, Excel, Power Point and ability to navigate web-based portals, databases, and forms)
• Other recommended experiences: Health Coaching, Community Health Worker certification. Interest in Motivational Interviewing, Cultural Competency and Health Literacy, Bridges out of Poverty program, Health Insurance enrollment.
