Job summary

Qualifications

• Excellent communication and detail-oriented.
• Ability to work independently and as part of a team.

• Valid Driver’s License with acceptable driving record.

Responsibilities

• Facilitate and reinforce skill development in social supports, including self-care, personal hygiene, relapse prevention, medication management, and planned community activities.
• Complete documentation, including progress notes and service plans, as required by regulations.
• Establish ongoing dialogue with treatment team, including staff, instructors, medical professionals, and family members to provide continuous support.
• Perform other duties related to enhancement of program service delivery.
