Qualifications

• High school diploma or GED required; 2 years’ experience in mental health preferred.
• Strong communication and detail-oriented.
• Ability to work independently and as part of a team.
• Proficiency with EHR, Outlook, MS Word, and Excel.
• Valid Driver’s License with acceptable driving record.

Responsibilities

• Support skill development in social supports, including self-care and community activities.
• Complete documentation, including progress notes and service plans, per regulations.
• Coordinate with treatment team, staff, instructors, clinicians, and family.
• Provide ongoing support and crisis prevention in the program.
• Perform other duties to enhance overall service delivery.
