Job summary

Support Management dba Community Intersections is seeking Residential Support Staff to assist adults with disabilities in their homes with daily living tasks, skills training, and community participation. You will work directly with 1–3 individuals, following their Service Plans and ensuring safe, respectful, and person-centered care.

Responsibilities include documenting progress, coordinating transportation, and supporting personal needs while maintaining a collaborative team approach.

Qualifications

• High School Diploma or GED required.
• Criminal background check required per state regulations.
• Driver's license and liability insurance required (where applicable).
• Three positive references, at least two from previous employers.
• Good written and verbal communication with the team.

Responsibilities

• Assist individuals with daily living activities such as cooking, cleaning, laundry, shopping, and personal care.
• Provide transportation coordination and community involvement support.
• Document daily notes and plan progress per the Individual Service Plan (ISSP).
• Support and encourage individuals to develop relationships in the community.
• Attend trainings and team meetings to stay current on programs and people served.
