Qualifications

• Must be at least 18 years old.
• High School Diploma or GED.
• Valid Driver's License.
• Previous experience in human services preferred.

Responsibilities

• Provide transportation for individuals to medical appointments, community activities, school/day programs, religious services, family visits, and other approved activities.
• Help individuals achieve goals outlined in ISP and BSP.
• Follow supervision requirements and support strategies in plans.
• Promote independence, choice, dignity, and community inclusion.
• Maintain a safe, clean, and supportive living environment.
• Complete required documentation (logs, charts, incident reports, etc.).
• Attend and participate in scheduled meetings and on-call duties.
