Job summary

Gateway BHS is seeking a dedicated Direct Support Professional to assist individuals in a Brunswick group home. You will support daily living activities, observe and document progress, and help implement individualized plans in a resident setting.

The role requires 21+ years of age, a HS diploma or GED, and possibly CNA or related experience. A valid Georgia driver’s license is required, with duties including transportation and community activities.

Qualifications

• Must be at least 21 years old.
• High School Diploma or GED equivalent.
• One year of direct care or social services experience or CNA/associate degree or two years of equivalent experience.
• Valid Georgia Driver’s License required.

Responsibilities

• Provide direct care and support in a residential setting.
• Assist with daily living activities, transportation, and medication assistance as needed.
• Develop and implement Individual Service Plans and treatment objectives.
• Promote community participation and independence.
• Maintain accurate progress records and documentation.
• Attend case reviews and coordinate with the care team.
