Job summary

Gateway BHS seeks a dedicated direct care worker to assist individuals in a 24-hour residential setting. You will support daily living activities, help with personal care, meal planning, transportation, and documentation of progress and behaviors.

Responsibilities include implementing Individual Service Plans, participating in meetings, and ensuring safety and cleanliness. A valid Georgia driver's license and ability to lift up to 50 pounds may be required. Training provided.

Qualifications

• Must be at least 21 years old.
• High School Diploma or GED required.
• 1 year experience in social/direct care or CNA certification preferred.
• CNA or associate degree qualifies for the role.
• Valid Georgia driver's license required.

Responsibilities

• Provide direct care and support in a residential setting.
• Assist with personal care, nutrition, and daily living activities.
• Implement Individual Service Plans and treatment objectives.
• Promote community participation and social engagement.
• Maintain accurate documentation and progress records.
• Provide transportation to appointments and community activities.
• Follow policies on Self-Administration of Medication.
• Report changes in condition to supervisors and safety hazards.
