• Ability to follow care plans and document progress accurately.

Responsibilities

• Support individuals with daily living skills, personal care, and routines.
• Provide transportation and accompany to medical appointments and community activities.
• Implement individualized support and behavioral plans, and document progress.
• Collaborate with families, other caregivers, and care providers.
