Qualifications

• High school diploma or equivalent.
• Valid driver's license.

Responsibilities

• Assist with daily living activities including medication administration, meal preparation, and personal care.
• Provide transportation to medical appointments and day programs.
• Help implement individualized behavioral and support plans.
• Accompany individuals on community outings and promote participation and independence.
• Maintain documentation and report health and safety concerns.
• Support individuals to achieve personal goals and meaningful participation.
