The role includes medication oversight, personal care assistance, and coordinating transportation for members in day programs and community activities.

The position requires 21+ years of age, a clean driving record, and the ability to obtain AZ fingerprint clearance.

Qualifications

• Must be at least 21 years of age.
• Current, unrestricted driver’s license with clean record.
• Ability to pass a criminal background check and obtain required clearances.

Responsibilities

• Supervise Direct Support Professionals and ensure adherence to plans and records.
• Oversee medication management and personal care assistance.
• Coordinate transportation, community activities, and scheduling.
• Maintain up‑to‑date files, incident reports, and ISP documentation.
• Provide guidance on budgeting, nutrition, and community participation.
