Home For Butterflies LLC. is seeking a detail-oriented Program Manager to oversee program and residential management services for 10–15 individuals with developmental disabilities in Manchester, NH.

The role emphasizes person-centered service delivery, documentation, and regulatory compliance. You will coordinate with individuals, families, guardians, providers and agencies to ensure high-quality services, health and safety, and individualized goals.

Responsibilities

• Manage a caseload of 10 to 15 individuals, ensuring services align with ISAs, goals and state regulations.
• Monitor the quality of residential and support services provided by Home Care Providers.
• Conduct monthly site visits to review progress toward goals and assess living environments.
• Provide backup Program Manager coverage to maintain continuity of services.
• Participate in individual-centered meetings, goal-setting and service planning.
• Maintain consistent communication with individuals, families, guardians, Area Agencies and service providers.
• Coordinate with behaviorists and healthcare professionals to support plans.
• Assist individuals with medical appointments and follow-up care.
• Complete and maintain accurate documentation including daily notes and reports.
• Ensure timely reporting and compliance with state regulations and agency policies.
• Support state inspections and implement corrective actions when necessary.
