Job summary

Conexio Care is seeking a Residential Services Associate for its mental health group homes. You will participate in treatment planning, coordinate medical and vocational services, and provide in-home support including medication assistance and ADL training.

Strong communication and crisis prevention skills are essential to help clients access safe, timely services. Two years’ experience with severe mental illness and a high school diploma are required.

Qualifications

• High School Diploma is required.
• At least two years of experience supporting individuals with severe and persistent mental illness.

Responsibilities

• Participates in treatment planning and case conferences with staff and partner agencies.
• Completes daily tasks and maintains therapeutic alliance with clients, linking to needed services.
• Coordinates medical and vocational services for clients.
• Supports independent living with housing, employment, and income resources.
• Provides in-home services including ADL training and medication assistance.
• Delivers supportive counseling and informs team of changes in client status.
• Responds to after-hours or emergency calls per program directions.
• Prepares and documents time worked and maintains confidentiality.
