Job summary

Hartford HealthCare Rushford Center in Rushford, CT, seeks staff for seasonal per-diem shifts to support recovery and independence. The role is designated Essential, with mandated coverage as needed and duties spanning community activities, documentation, and safety.

Responsibilities include guiding clients in daily activities, completing regulatory records, coordinating with families and providers, and managing medications and inventories in a supportive residential setting.

Qualifications

• Associate's degree in Human Services or related field.
• Bachelor's degree in Human Services or related field preferred.
• Two years of experience with individuals with mental illness/substance abuse.
• Valid driver's license and CPI certification within 90 days.

Responsibilities

• Provide guidance and accompany clients during community activities to improve independence and social functioning.
• Complete EPIC progress notes, DDAP documentation, ROI, and admission/discharge paperwork.
• Communicate with management, family, and providers about treatment plans and progress.
• Use CPI crisis intervention to maintain safety in residential settings and teach life skills.
• Transport clients to medical appointments and recovery activities using HHC vehicles.
• Manage medication acquisition, storage and inventory, and observe self-administration.
