Hartford HealthCare's Rushford Center is seeking a qualified individual to support residents with mental health and substance use needs. The role involves guiding clients during community activities, completing required documentation, and coordinating with treatment teams to ensure safe, recovery-oriented care.

Essential duties include crisis intervention, medication management, and documentation in EPIC/DDAP systems, with shifts that may be per-diem.

Qualifikationen

• Associate's degree in Human Services or related field required
• Bachelor's degree in Human Services or related field preferred
• 2 years of work experience with individuals with mental illness and/or substance abuse
• 4 years of work experience with individuals with mental illness and/or substance abuse preferred
• Valid driver's license required
• Crisis Prevention Institute (CPI) certification within 90 days of hire
• Able to read, write and speak English
• Basic working knowledge of computers, email, software applications, and typing skills

Aufgaben

• Provides guidance and accompaniment to clients during community activities to enhance independence and social functioning.
• Completes EPIC progress notes, DDAP documentation, and related paperwork.
• Communicates with management, conservators, family, and other supports about treatment plans and progress.
• Utilizes crisis intervention skills to maintain safety in a residential setting and teach life skills.
• Uses HHC vehicles to transport clients to medical and social activities.
• Responsible for acquiring, storing, and inventorying medications and observing self-administration.
