Job summary

Thomas Allen in Minnesota is seeking a Residential Assistant Program Manager to work on-site in Chaska. You will collaborate with the Program Manager to ensure day-to-day operations and person-centered care under 245D regulations.

The role emphasizes leadership, documentation, and coordination of services, with duties including scheduling, training, and ensuring access to medical and behavioral supports in a safe environment.

Qualifications

• Must have at least 2 years of full-time experience in a Community Residential setting.
• Ability to coordinate with leadership to develop individualized service plans.
• Must be able to assist with personal hygiene, meals, home maintenance, and community outings.
• Must maintain accurate documentation and comply with DHS regulations.
• Must pass Thomas Allen's Medication Administration training and prove competency.
• Must have valid driver’s license and a clean driving record.

Responsibilities

• Partner with leadership to lead a team in providing person-centered support.
• Collaborate to develop and implement individualized service plans reflecting the needs of individuals served.
• Ensure access to quality medical and behavioral supports in a safe environment.
• Maintain documentation and comply with regulations and policies; uphold professionalism and ethics.
• Coordinate transportation services as needed, promoting inclusivity.
• Participate in quality assurance and improvement activities.
• Provide training and coaching to team members.
• Assist with scheduling, including alternating on-call duties with leadership.
• Perform other duties to support team goals.
