Job summary

Vinfen is seeking an ACCS Residential Nurse (RN) to promote community integration for adults with mental health conditions and complex medical needs in a residential setting. You will coordinate access to healthcare with care coordination entities and medical/behavioral health providers, monitor medical and psychiatric conditions, and lead health promotion strategies for residents under MAP compliance.

Qualifications

• Licensed RN in Massachusetts.
• Bachelor's degree preferred.
• Experience with psychiatric disability or co-occurring disorders preferred.

Responsibilities

• Oversee nursing services for residents with complex medical and psychiatric needs.
• Coordinate health care access with care coordination entities and providers.
• Ensure MAP compliance, medication storage, administration, and disposal per policy.
• Conduct regular nursing assessments and update care plans as health status changes.
• Train staff on health promotion, disease management, and recovery-oriented approaches.
• Provide on-call clinical coverage as scheduled.
