Job summary

Comunilife Inc. in New York's Gramercy Residence Program seeks a Mental Health Counselor to provide supportive services for clients, conduct psychosocial assessments, and help develop and implement service plans that support recovery, vocational and interpersonal goals.

The role requires a Master's degree in Social Work or Mental Health Counseling, at least two years' experience, and strong communication skills. Bilingual English/Spanish is a plus; comprehensive benefits are offered.

Qualifications

• Master's degree in Social Work or Mental Health Counseling.
• Two years' work experience.
• Experience with HIV/AIDS, chronic mental illness, substance abuse a plus.
• Bilingual English/Spanish a plus.
• Knowledge of treatment and recovery for HIV/AIDS, SMI, homelessness.
• Excellent oral and written communication; MS Office/Google proficiency.

Responsibilities

• Obtain consent and maintain collateral contacts with providers.
• Complete psychosocial assessments within 30 days of placement.
• Develop service plan goals based on psychosocial recommendations.
• Coordinate follow-up and monitor progress during visits.
• Advocate for tenants and participate in discharge planning and care meetings.
• Document all services in AWARDS and maintain compliance with requirements.
