Comunilife is looking for experienced Case Managers to fill open positions for their Manhattan HRA congregate Residence Program, The Gramercy Residential Program. The Case Manager is responsible to assess the resident's needs and develop a case plan to ensure that the resident is linked to needed services both inside the residence and in the community. Assist residents in developing necessary skills to live more independently in permanent housing. Responsible for general case management, working with resident’s families and significant others providing congregate services, i.e., case management and recreational activities to persons living with HIV/AIDS. Other responsibilities include working harmoniously with Gramercy Residential Program staff in accordance with Comunilife principles and guidelines.

Bachelor's Degree in Social Work, Public Administration preferred. Associates degree with at least 4 years of related experience or High School degree with at least 6 years of case management experience may be accepted. Individual, family and group counseling experience required, particularly with mentally ill populations. Preference given to persons with experience in working with homeless individuals in Congregate Programs, bi-lingual, bi-cultural in Spanish, a plus.
