Job summary

LifeStream is seeking full-time and part-time Licensed Practical Nurses (LPNs) in Massachusetts to support individuals with disabilities at home and in the community. A current LPN license, valid driver’s license, vehicle access, and the ability to pass screening are required. Starting pay is $36/hour with weekend differentials, and a comprehensive benefits package is offered.

No prior experience is required to qualify. Shifts include day and evening options to accommodate staff needs.

Qualifications

• Licensed Practical Nurse with current MA licensure.
• Valid driver’s license and access to a vehicle.
• Must pass employment screening including CORI/fingerprinting.

Responsibilities

• Assess individuals’ medical regimes and ensure access to health services.
• Provide direct nursing care to maintain high standards of care.
• Maintain medical charts, medication ordering and inventory.
• Review doctor’s orders for accuracy and comply with regulations.
• Adhere to DPH regulations for medication storage, administration and destruction.
• Assess daily well-being and respond to consumer needs.

ESSENTIAL DUTIES AND RESPONSIBLITIES: Assess all individual?s medical regimes to ensure that all individuals are referred for and receive health services, which will maximize health and safety. Ensures that all consumers are referred for and receive services which assist with achieving set goals and maximize physical and emotional well being. Provide direct care duties to maintain the highest level of care standards. Maintenance of medical charts, medication ordering regime and other medical supply inventory. Assess all consumer complaints and overall well being of each individual on a daily basis. Review doctor?s orders for accuracy Adheres to DPH regulations for medication storage, administration and destruction. QUALIFICATIONS AND REQUIREMENTS: Maintenance of an acceptable CORI and fingerprinting Valid Driver?s license Negative drug screen Use of personal vehicle for work purposes Commitment to the philosophy of community inclusion EDUCATIONS and/or EXPERIENCE: Licensed Practical nurse with current licensure in Commonwealth of Massachusetts. Demonstrated history of quality work performance and teamwork in previous positions. Familiarity with the needs of individuals with developmental disabilities and brain injury.
