Job summary

LifeStream in Massachusetts is seeking a Lead LPN to oversee day-to-day nursing care in assigned homes and support clinical operations. The role reports to the Residential RN Coordinator and focuses on coordinating health care, maintaining charts, and serving as a clinical resource for staff.

Starting pay is $37/hour with a 40-hour schedule (Monday–Friday plus varying shifts). A $3,000 sign-on bonus is paid at 60, 120, and 180 days.

Qualifications

  • Licensed Practical Nurse (LPN) license required.
  • Strong clinical coordination skills.

Responsibilities

  • Coordinate health care and health maintenance for individuals in agency programs.
  • Provide direct training and support to staff on clinical practices.
  • Maintain medical charts, medication orders, and medical supply inventory.
  • Ensure timely delivery of health services and assist with scheduling and documentation.

  • Health Care Coordination: Assess all individual's medical regimes to ensure that all individuals are referred for and receive health services, which will maximize health and safety. Ensures that all consumers are referred for and receive services which assist with achieving set goals and maximize physical and emotional well-being. Provide direct care duties to maintain the highest level of care standards. Maintenance of medical charts, medication ordering regime and other medical supply inventory
  • Training Identify areas of clinical development needed for program staff and coordinate trainings to address any needs identified. Participate in the training of new LPN hires per the discretion of RN, consistent with Nurse Mentorship Program.
  • Quality Assurance Attend all required meetings and maintain required licensures. Coordinate health care needs of individuals served and assure that delivery of these services occurs in a timely fashion. Work scheduled floor hours. Accompany all consumers to doctor?s appointments when needed. Review medication administration systems on a monthly basis with the development of an action plan, to be completed with Residential Manager in a timely manner.
  • Administrative Ensure that all required medical documentation is completed within a timely fashion and according to established guidelines and reflect the progress and interest of each individual served. Maintain open and effective communication with DDS/MRC Service Coordinators, parents, guardians, employees, other managers, vendors, and community. Effective communication of all programs needs reported to Residential Coordinator and Nursing Coordinator in a timely manner. Report any suspicions of abuse or neglect to DPPC as a mandated reporter.
