Qualifications

• Excellent verbal and written communication skills.

Responsibilities

• Assess individuals’ nursing needs and ensure health and safety.
• Provide prescribed medical treatments and personal care in home settings.
• Administer and store medications and medical supplies safely.
• Conduct health assessments and monitor health conditions regularly.
• Coordinate care with Speech, Occupational, and Physical Therapists.
• Document progress notes and nursing assessments.
• Assist with daily living activities and health education.
• Attend trainings, participate in meetings, and perform other duties as assigned.

Lifeworks is hiring a part time Licensed Practical Nurse (LPN) to work with Individuals with ID/DD in one of our Residential Programs. We are a nonprofit human service agency that provides adults with developmental disabilities the opportunity to live engaged and meaningful lives. We're searching for an enthusiastic individual who will bring their personality, an independent work ethic, and a strong belief in enhancing the lives of those we serve.

Essential Duties

• Ensures the health and safety of individuals by assessing individuals for specific nursing needs.
• Provides prescribed medical treatment and personal care to individuals in home settings, performing the following duties.
• Administer and safely store all medical supplies and prescribed medications.
• Treat any daily health care problems, including first aid and emergency problems.
• Regularly assess clients for specific nursing needs, and/or health problems and monitor accordingly.
• Lift and transfer individuals as appropriate to their needs.
• Provide preventative health maintenance: blood pressure check, weight, health care education.
• Participate in or complete nursing assessments, progress notes, and other applicable documentation.
• Assist in coordination of health care services with Speech Therapist, Occupational Therapist and Physical Therapist.
• Provide individual and small group instruction in self-care, activities of daily living, communication, social and motor skills.
• Comply with all applicable State and Federal regulations, agency policies and procedures and personnel practices.
• Act as resource person to program staff, clients, families and care providers regarding health care issues and concerns.
• Make home visits when necessary.
• Attend training sessions and staff meetings as required.
• Perform other duties as requested by supervisors.

Requirements

• Current license with the Massachusetts Board of Registration in Nursing and one year of experience.
• Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
• Experience working with Individuals with ID/DD
• Valid Driver's License required
• Valid MAP certification is preferred
• Excellent verbal and written communication skills
