Job summary

You will help individuals with daily living activities, community participation, and personal goal attainment while promoting safety and well-being. The role emphasizes advocacy, trauma-informed practices, and a commitment to diversity, equity, and inclusion.

Qualifications

• High School Diploma, GED or equivalent, required.
• Valid Driver’s License and acceptable driving record, required.
• Reliability, willingness to learn, and openness to new opportunities.

Responsibilities

• Assist in developing person-centered service plans and support their implementation.
• Provide daily living support, skill development, and help achieve personal goals.
• Assist individuals with transportation and access to community resources.
• Promote community inclusion, relationship building, and social role development.
• Maintain a safe, clean and supportive environment for individuals served.
• Complete and maintain required documentation accurately and timely.
