Job summary

Community Partners NH is seeking a Case Management Supervisor to oversee a Permanent Supportive Housing program. You will collaborate with housing authorities, social service agencies, and landlords to create stable, affordable housing opportunities with subsidies such as Section-8 for adults with serious mental illness.

The ideal candidate has a Bachelor’s degree in social sciences or human services and at least one year of relevant experience, with strong communication skills and a valid

  • One year experience working with special needs populations; education may substitute.
  • Strong written and oral communication skills.
  • Ability to use technology provided to perform job requirements.
  • Valid driver’s license and reliable transportation required.

Responsibilities

  • Implement a Permanent Supportive Housing program for eligible adults with serious mental illness.
  • Collaborate with local housing authorities, social service agencies, and landlords to secure stable, affordable housing.
  • Coordinate with subsidies such as Section-8 or other permanent housing subsidies.
  • Manage a caseload of clients residing in Strafford County receiving Bridge subsidies.
  • Provide on-call support and respond to after-hours concerns as needed.

The Supported Housing Specialist is responsible for implementing a Permanent Supportive Housing program to support eligible adults with a serious and persistent mental illness by working collaboratively with local housing authorities, other social service agencies, and property owners/landlords to develop and provide stable, affordable, and long-term independent housing opportunities through short and long-term subsidies such as Section-8 vouchers or other types of permanent housing. Works with a caseload of clients who reside in Strafford County who receive the Bridge's subsidy. These individuals may or may not be receiving mental health services from community partners.

Our Ideal Candidate Will Have The Following Attributes
  • Bachelor’s degree preferred, preferably in the social sciences or human services field.
  • One year experience working with special needs populations; experience can be substituted for formal education
  • Ability to work flexible hours to provide on-call support and respond to after-hours concerns and emergencies
  • Ability to communicate effectively both orally and in writing
  • Ability to utilize technology provided to perform requirements of the job
  • Valid driver’s license, proof of adequate automobile liability insurance and reliable transportation required
