Rockland Hospital Guild Clue is seeking a Case Manager to oversee day-to-day residential program operations, including medication management and staff coordination. The role focuses on maintaining a safe, clean living environment and scheduling activities to support residents’ progress.

You will document resident interactions, manage appointments, and plan weekly shopping trips, while ensuring compliance with house policies and safety standards.

Responsibilities

• Count and verify controlled medications; ensure each resident has sufficient supply and no missing meds.
• Track residents' whereabouts to ensure everyone is present or accounted for; monitor house safety.
• Check personal hygiene and cleanliness in rooms; assist residents with cleaning as needed.
• Schedule daily activities using restorative services to help residents advance to independent living.
• Document conversations and appointments; fax doctor orders to pharmacies; update contact info in system.
• Arrange 1–2 weekly shopping trips for residents to buy household supplies and food.
• Coordinate evening/weekend activities with supervisor; ensure funds are available for outings.
• Inspect rooms for daily living items (sheets, towels, toiletries) and confirm working house appliances.
• Prepare houses for daily operations; ensure all items and systems are in working order.
• Evening/weekend coverage may be required; assign resident chores to maintain cleanliness.
