We partner with clients and the community to promote recovery from mental illness and substance use, treating more than 7,500 adults and children each year. CMHA is Connecticut’s first fully Joint Commission accredited Behavioral Health Home and is a SAMHSA Certified Community Behavioral Health Clinic (CCBHC).

Position Summary

Part‑time 2nd shift Respite Case Manager. The role is located at 66 Clinic Dr., New Britain, CT 06501, and operational hours are Saturday‑Sunday 3:30 pm‑11:30 pm.

Essential Responsibilities
  • Maintain an appropriate caseload as determined by CMHA and the funder.
  • Provide direct care to clients, including individualized and group education, skill building, recreation, and household maintenance activities, and monitor the environment for cleanliness, safety, and repairs.
  • Teach life skills necessary for independent living (e.g., finances, entitlements, daily living activities, nutrition, housing maintenance, and repair).
  • Educate clients to utilize emergency and community resources to meet basic needs.
  • Develop and review individualized treatment plans with clients to identify goals and track progress.
  • Ensure timely documentation in accordance with CMHA policies and procedures (progress notes, treatment plans, authorizations, program orientation, consent to treat, monthly targeted case management notes, shift reports).
  • If applicable, supervise and document medication self‑administration according to program policy.
  • Collaborate with other CMHA teams to ensure seamless service communication.
  • Communicate with external providers to ensure consistent, accessible services; provide transportation as needed.
  • Perform other related duties as assigned.

Qualifications

Education: Bachelor’s Degree in a mental health‑related field preferred or equivalent experience.

Credentials and Licenses:

  • Valid Connecticut driver’s license and reliable vehicle.
  • Will become certified in MANDT, First Aid, and CPR.
  • Participate in annual trainings as assigned.

Experience:

  • Minimum of one year in case management or a related field with a solid understanding of mental health and co‑occurring conditions.
  • Beginner to intermediate experience with Microsoft Office (Word, Outlook, Excel, PowerPoint).
  • Experience or willingness to learn an Electronic Medical Record (EMR) system (appointment scheduling, authorization tracking, assessments, treatment plans, progress notes, discharge summaries).

  • Incorporate recovery‑based treatment and language into documentation and daily operations.

  • Work flexible schedule when needed.
  • Respond effectively to emergency situations.
  • Design, deliver, and ensure highly individualized services; manage time and priorities effectively; maintain client confidentiality per CMHA policy and regulations.
