Sample Supports is seeking a Residential Coordinator to oversee host home placements, family caregivers, and adults in supported apartment programs. The role blends autonomy with advocacy and a strong focus on building real connections.

You will coordinate services, monitor care quality, and ensure compliance with relevant licensing and documentation requirements while engaging with interdisciplinary teams to support person-centered plans.

Qualifications

• Oversee a caseload of adults receiving residential services with emphasis on health and safety.
• Ensure compliance with licensing regulations and documentation standards.
• Collaborate with case managers, guardians, therapists and advocates.
• Maintain and audit client records with timely reporting.
• Respond to client needs with professional judgment and care.

Responsibilities

• Manage a caseload of adults receiving residential services, ensuring their health, safety, and quality of life
• Oversee care coordination and compliance with licensing regulations
• Monitor medical needs and ensure follow-up on protocols and PRN medications
• Collaborate with interdisciplinary teams including case managers, guardians, therapists, and advocates
• Attend and contribute to clinical meetings and related reviews
• Maintain accurate documentation and complete required reporting
• Conduct unannounced home visits and safety welfare checks
• Support development of person-centered treatment and behavior plans
• Communicate regularly with clients and providers to build relationships
• Use professional discretion to solve problems and manage crises
