Job summary

Center for Human Development (CHD) in East Longmeadow, MA, is seeking a Clinician Master’s / Clinical Care Manager for Heron House, an 18-bed residential program. The role integrates psychiatric treatment, nursing care, and direct support, delivering psychotherapy and comprehensive care management.

The position requires an MSW or equivalent, MA licensure eligibility or licensing preferred, and experience with adults with complex health needs. Competitive salary and robust benefits are offered.

Qualifications

• Master’s Degree in Social Work or related behavioral health field.
• License-eligible or licensed in Massachusetts (LICSW/LCSW/LMHC preferred).
• Experience with adults with serious mental illness in residential or integrated care settings.

Responsibilities

• Deliver comprehensive assessments and individual psychotherapy.
• Coordinate treatment across nursing, psychiatric and residential teams; liaise with external providers.
• Develop and update individualized treatment plans in the EHR; ensure compliance with standards.
• Lead care coordination and crisis intervention support within the interdisciplinary team.
• Assist with discharge planning and community reintegration.
