Responsibilities

• Coordinate residential treatment referrals and admissions for out-of-home behavioral health services.
• Identify clinically appropriate residential providers to meet treatment goals.
• Manage referrals, track responses, and document facility activities.
• Collaborate with behavioral health homes, providers, and community partners.
• Monitor referral status to promote timely access and minimize delays.
• Serve as primary contact for providers and internal stakeholders.
• Respond urgently to disruptions or changes by coordinating alternatives.
• Participate in utilization management rounds and case reviews.
• Facilitate meetings with providers to improve outcomes.
• Support continuity of care with Care Management and other departments.
