You will manage referrals, monitor timelines, support complex facility challenges, and collaborate across multidisciplinary teams to promote quality outcomes and appropriate resource use.

Qualifications

• Bachelor's degree in Behavioral Health, Psychology, Social Work, Healthcare Administration, or related field.
• Experience in behavioral health utilization management, case management, discharge planning, or residential treatment preferred.
• Willingness to travel up to 10% within Arizona.

Responsibilities

• Coordinate residential treatment facility referrals and admissions for members approved for out-of-home behavioral health residential treatment.
• Identify clinically appropriate participating residential treatment providers to support treatment goals.
• Manage referral processes, track provider responses, and maintain documentation.
• Collaborate with behavioral health homes, treating providers and partners to ensure timely submission of information.
• Monitor referral status and facility timelines to promote timely access to care.
• Serve as primary contact for providers and internal stakeholders with facility-related questions.
• Respond urgently to facility disruptions or changes by coordinating alternatives.
• Participate in utilization management rounds and case reviews to support team discussions.
• Facilitate meetings with residential providers to improve collaboration and outcomes.
• Partner with Care Management, Utilization Management, Quality Management to support continuity of care.
• Maintain accurate case documentation and tracking in line with policies.
