Job summary

Acenda Integrated Health is seeking a PRN Residential Assistant in New Jersey to support mothers and children in a recovery-focused program. You will provide 24/7 assistance, supervision, and connections to community resources within the A Place to Call Home program.

The role requires a High School diploma or equivalent plus at least one year in a recovery-oriented setting, knowledge of AA steps, and a valid driver's license with insured vehicle. Bilingual abilities are a plus.

Qualifications

• High School degree or equivalent plus at least one year of experience in a Recovery-oriented role.
• Knowledge of the 12 steps for AA.
• Must have and maintain a valid driver's license, insured vehicle, and clean driving record.
• Professionalism and a commitment to excellence in care.
• Bilingual abilities considered a plus.

Responsibilities

• Work directly with mothers and children and assist with connection to community resources.
• Provide support in recognizing cravings, urges, triggers; and provide skill building including relapse prevention planning.
• Follow up with mothers up to six months post discharge, and provide links to supports as appropriate; and document.
• Prevent and assist with crisis situations; help reduce potential for relapse.
• Engagement and coordination with integrated team members for continuity of care, including written daily report when on shift.
• Record all documentation (UDS, med administration, individual sessions, group sessions, interventions, collateral phone contacts, and as needed releases of information, emergency contacts etc), in the EHR for continuity of resident records.
• Scan all written documentation into resident EHR (outside provider Urine Drug Screen, discharge paperwork, important documents, etc.).
