• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services.
• Develop and coordinate department policies; review annually and suggest changes.
• Provide information on Medicare/Medicaid and financial assistance options.
• Maintain social service progress notes and coordinate with other departments.
• Assist with discharge planning and community resources as needed.
• Support resident independence, well-being, and coping with illness.
